WorkSafeBC investigation wraps up
WorkSafeBC has concluded an investigation into the death of a worker at a highrise construction site in Vancouver on Dec. 2, 2010. Orders have been issued to HP Construction, the direct employer of the deceased worker and Ted Newell Associates Inc., the engineering firm involved in the project.
Penalties are being considered against both firms.
The incident took place at 3 Harbour Green.
A worker was climbing walers on the side of a concrete wall form to access a work platform.
The form fell and the worker was able to jump clear, but another worker was struck by the falling 1,260 kilogram form and suffered fatal injuries.
The investigation determined that a lack of site-specific design drawings, ineffective nailing and an unsafe procedure to climb wall forms all contributed.
